Long-Term Care Services

Medicaid Long-Term Care Services can provide help with your care needs in the setting of your choice. Options include care in your home, Adult Foster Homes, Residential Care Facilities, Assisted Living Facilities, and Nursing Homes. Medicaid is available to older adults (age 65 and over) and adults with physical disabilities (age 18 and over) who need help with tasks of everyday living.

Adult Foster Care

Adult foster care is an excellent choice for someone looking for an affordable, homelike setting. Informal adult foster care has been a part of our society for years. People that were unable to maintain their health while living alone moved in with family, friends, and neighbors. In an adult foster home, medical and personal care are provided in a manner that encourages maximum independence and enhances the quality of life.

Public Disclosure File

Senior & Disability Services maintains a public disclosure file of complaints involving residents in licensed facilities. The disclosure file also contains information about a facility's license history. Please contact our office at 541-682-4038 if you are interested in viewing the public file.

 

Case Management Services

Case Managers can help older adults and adults with disabilities who need personal care assistance to access the services and resources they need. Services that may be available under this benefit include:

  • A visit to your home to assess your needs and develop a care plan.
  • Referral for other community-based services as needed.
  • Arrangement for you to receive supportive services such as home-delivered meals or transportation to medical appointments.
  • Regular contact with you to ensure that the services you are receiving continue to meet your needs.

 

Oregon Project Independence - Medicaid (OPI-M)

In the Spring of 2025, a new OPI-M program opened for adults over the age of 60 and adults with disabilities age 18-59.

OPI-M provides limited in-home services to people who need a little help to continue living independently in their own homes. People who earn incomes too high to qualify for Oregon Health Plan (OHP/Medicaid) may qualify for OPI-M because its income limit is much higher than traditional OHP income limits. Services offered through OPI-M may include in-home care services, Meals on Wheels, case management, home modifications, and assistive technology supports, among other things. Additionally, OPI-M benefits will not be subject to estate recovery, which is when the assets an individual leaves behind after their death are used to pay back the government for certain services. Eliminating estate recovery strengthens the financial stability of families by making it possible for generations to pass down life savings.
