Services and Programs Available through Senior Connections

Senior Connections work to help seniors continue to live independently in their own homes. Senior Connections staff, known as Area Coordinators, are stationed at our offices across six Lane County communities, including Florence, Oakridge, Junction City, Veneta, Cottage Grove, and Eugene. Your local Area Coordinator can provide you with information about local, state, and federal services, and the benefits available to seniors and family caregivers.

We can connect you to services such as:

  • Options Counseling
  • Energy assistance (LIHEAP, rural areas only)
  • Meals on Wheels
  • In-home support
  • Rural medical transportation
  • And so much more!

Services are available to all seniors age 60 or over, or their family caregivers. Services are available free of charge, regardless of income.

 

Family Caregiver Support Program

Area Coordinators can help unpaid caregivers access the services and resources they need. Services provided will:

  • Meet the needs of the caregiver and enhance support given to the care recipient.
  • Help the caregiver become a better advocate and more confident in assisting the care recipient with their physical, cognitive, and behavioral needs.
  • Deter placement in a long-term care setting when feasible and promote continued care within the home and/or in alternative community settings for care recipients for as long as possible or desirable.
  • Help provide support to older relatives who are caregivers for children.

Eligibility

This program is open to any unpaid caregiver of:

  • An individual with Alzheimer's disease or other dementia of any age.
  • An adult over the age of 60.
  • A parent or older relative caregiver age 55 or older who lives with, and is the primary caregiver, for an individual with disabilities between the ages of 18 and 59.
  • An older relative caregiver (other than a parent) age 55 or older who lives with, and is the primary caregiver for, a child age 18 or younger.

 

Healthy Aging and Evidence-Based Health Promotion

Living Well

The Living Well Program is a no cost, six-week skill building workshop that teaches practical ways to live a healthy lifestyle. Participants will learn skills that will help you take charge of your health by: 

  • Improving communication
  • Problem solving
  • Action planning
  • Decision making

Gain control and feel better by learning with a small group of peers, facilitated by a peer leader. 

What Does the Program Consist Of?

  • Small supportive group of 5-12 people who are also experiencing a chronic condition
  • The six-week workshop is held once a week for 2.5 hours
  • Free copy of the companion book, Living a Healthy Life with Chronic Conditions
  • A facilitator that has been certified by Stanford University's Chronic Disease Self-Management Programs

Oregon Money Management Program (OMMP)

The Oregon Money Management Program (OMMP) offers free services to seniors and people with disabilities who have limited resources. This program promotes independent living and self-sufficiency while protecting against financial abuse and fraud. This program does not manage private pensions or other resources.

Eligibility

Individuals 60 and older with an ongoing Case Manager/Care Coordinator are eligible for our services. Individuals 18 and older are eligible with an ongoing Case Manager/Care Coordinator and a documented disability.

Services Offered

Representative Payee

For individuals who receive federal benefits such as Social Security or Veterans Benefits and are unable to manage their federal benefits on their own.

Bill-Pay

For individuals who remain in control of their personal finances but need help keeping on track. Bill-pay service can be short-term or long-term based on an individual's needs.

The Aging and Disability Resource Connection, or ADRC, is a free service that offers the public a single source of information and referral for older people, people with disabilities, and their caregivers, regardless of their income. ADRC is dedicated to:

  • Identifying community resources to meet short-term or immediate needs.
  • Providing resources and services that support the needs of family caregivers.
  • Helping individuals to understand the various long-term care options available to them.
  • Enabling individuals to make informed, cost-effective decisions about long-term care.
